The OHIP-covered family practice services provided by the practice include the following:

• Acute illness diagnosis and management
• Chronic disease management
• Antenatal care
• Infant care
• Adult and child Periodic Health Exams
• Preventive Care:
• Blood pressure screening
• Cholesterol screening
• Bone density screening
• Routine cancer screening (Breast, Cervix, Colon)
• Immunizations
• Weight management
• Mental illness diagnosis and treatment
• Pre-operative assessments
• Smoking cessation
• Office procedures:
• Lesion removal
• Skin biopsies
• Cryotherapy
• Joint injections

Prescription Renewals

Renewals of regular prescriptions can be requested through your pharmacy. If you have no repeats remaining on your prescription, your pharmacy will fax our clinic requesting a renewal. You must provide 72 hours for a faxed prescription renewal, although these are typically processed within 24 hours.

Faxed prescription renewals are not covered by OHIP and many practices charge approximately $20 per prescription to provide this service. Currently, I am pleased to offer this service to enrolled patients free of charge and waive all fees associated with faxed prescription renewals.

Anesthesia

As a GP Anesthetist, the following anesthesia services are provided:

• Operative anesthesia at Stratford General and Clinton Public Hospitals
• Epidural corticosteroid injections

Uninsured Services

Certain services are not covered by OHIP. These include the completion of insurance and government forms, sick notes and back to work notes, drivers medical examinations, pre-employment examinations, travel immunizations, and copies of medical records.
